To All St. Frances Cabrini Faith Community Members:
I encourage you to contact me for individual consultation on medical and spiritual needs. This is free and confidential. I am available on Wednesday mornings from 9:00AM to 11:00AM, or Friday mornings (with the exception of First Fridays) from 10:00AM to 12Noon. If these times do not suit your schedule, I would be happy to schedule a time that is most convenient for you. My office is located on the Saint Frances Cabrini Worship Site campus in the former convent.
Information will be added here from time to time to help improve body, mind, and spirit; increase knowledge on inspirational or medical resources, empower you to advocate for yourself or a loved one, locate resources or enrich your spiritual life to grow closer to Jesus.

More than 35 million families have used Five Wishes to capture their wishes. Five Wishes has changed the way we talk about advance care planning by ensuring that it is not just an end of life planning tool, but the beginning of an important family conversation.
It's written in everyday language, making it easy to understand and complete.
It covers personal, spiritual, medical and legal wishes all in one document.
It allows your family or caregiver to know exactly what you want, relieving them from the difficult position of guessing your wishes.
Five Wishes is legally valid in most states. The document includes all the instructions and information that you need to create a valid advance directive. We know from countless stories from families that have benefited from Five Wishes that it usually starts with one person who learns about the program, completes it, and then shares it with his or her entire family. Be the person who shares this gift with those you love. Start your conversation with one of the programs below or select a product to learn more.
It's more than just a document. Five Wishes is a complete approach to discussing and documenting your care and comfort choices. It's about connecting families, communicating with healthcare providers, and showing your community what it means to care for one another.
